It's not nearly as slow this time- Ali can tell Concord is taking this well- but it's a movement with purpose. The book said he's supposed to spread this oil around inside, and Ali is taking his job seriously. He might be more tense than Concord here, even.
He slides and spreads his fingers a bit, trying to ease Concord open without causing too much pain. He knows there will be discomfort no matter how slow he goes, but he doesn't want Concord to be in pain, even if the man can take it. He's still treating Concord like something precious. And to further that idea, he slips his free hand up to Concord's thigh and starts massaging it gently, trying to give Concord some pleasant feelings along with the stretch.]
